Locke Family Medicine is working towards becoming a Patient Centered Medical Home.

What does this mean?

The doctors at Locke Family Medicine participate in the Patient Centered Medical Home (PCMH) program sponsored by the National Committee for Quality Assurance.

The Patient Centered Medical Home is a care delivery model whereby patient treatment is coordinated through their primary care physician to ensure they receive the necessary care when and where they need it, in a manner they can understand.

The objective is to have a centralized setting that facilitates partnerships between individual patients, and their personal physicians, and when appropriate, the patient’s family. Care is facilitated by registries, information technology, health information exchange and other means to assure that patients get the indicated care when and where they need and want it in a culturally and linguistically appropriate manner.

By providing a Medical Home for our patients, our goal is to develop a partnership between the patient and healthcare providers. Our office team will help our patients arrange appropriate care with other qualified providers and community resources. We will help our patients coordinate care with specialists, hospitals, health agencies, nursing homes and other health facilities/health professionals in the community.

PCMH recognized doctors advocate for their patients and commit to attaining optimal Patient-Centered outcomes by improving communication with their patients, opening and improving their scheduling of appointments, increasing care coordination and empowering patients to take more responsibility for their health.

To provide you with the best healthcare, it is important that the Locke Family Medicine care team understand your medical and mental health history, your living situation, relationship status and employment history.  Please be prepared with a list of current medications you take including doses and any allergies you have.  Please provide your care team with the names and contact information for any other medical providers that you have seen so that we may better coordinate your care.

We understand the advantage of treating the whole person in our Medical Home and since your personal family physician is the center and beginning of most of your medical care, we will help you coordinate your care among all treatment providers.
